[emphasis theirs] You can see it at the local police office all the way to the Supreme Court. The only way to deal with these inevitable problems is to keep exposing and fixing them. They will always rearise. I might add even interpersonal relations can suffer from a similar problem of ever-accruing small mistakes growing into huge, unfixable ones.
